Student Registration
Welcome to the Upper Freehold Regional School District. We are committed to the whole student - providing an exceptional education and a next level experience for all students.
Preschool & Kindergarten students for Fall of 2025, please check back on this page in February for updates! (more detail below)
We are pleased to offer online pre-registration to streamline the registration process. After completing the online pre-registration and submitting the required documentation, you will be contacted via phone or email to complete the registration process. If you are unable to use our online system, please contact me to schedule an appointment. Registrations are by appointment only.
As a reminder, all supporting documents must be uploaded to complete the registration process.
Registration Document Checklist
Please prepare the following documents to complete the registration process:
Student’s Proof of age
Original birth record (with official raised seal)
Valid passport
Authorization for Record Release Form
Additional Student Background Information Form
Summary of School Residency Law Form
McKinney Vento Form
FERPA Acknowledgement Form
Language Survey Form
NJSIAA Form (Grades 9-12)
Health Records
Immunization Records - Current records can be obtained from previous school or pediatrician. These must be current with dates and translated into English.
Physical Examination ( within the past 365 days from the child’s first day of school)
Universal Child Health Record - can be obtained from previous NJ School.
Proof of Residency*
One of the following is are required to register a student:
Homeowner - property tax bill, contract of sale, or deed
Tenant - Current lease with expiration date; if residing as a tenant without a lease, a signed, notarized sworn statement of tenancy (affidavit) completed by the landlord.
Millstone Residents (9-12): contact Millstone Board of Education at 732-786-0950 ext 5130 for an affidavit of residency. This form, will be uploaded under the "proof residency" link.

Child is domiciled with a district resident other than the parent – signed, notarized sworn Statement of Residency (Affidavit) completed by the district resident and parent / guardian. In this scenario, guardianship verification of the district resident is also required.
Child and parent living with district resident - signed, notarized sworn Statement of Residency (Affidavit) completed by the district resident and parent / guardian.
Child placed by court - court order placing child in home of district resident.
Child placed by Child Welfare Agency - document of child welfare agency ordering that child be placed in home of a district resident.
Two of the following documents containing a district address:
Driver's license
Car registration
Any insurance documents
Current utility bill
Mortgage statement
Official mail (bank statement, government correspondence: Internal Revenue, Division of Taxation, Social Security Administration)
Income tax return
*McKinney Vento Homeless Assistance Act of 2001. If you lack a fixed, regular and adequate nighttime residence, please notify the Registrar.
Academic Records (transcript if grades 9-12; latest report card if grades 1-8)
Special Education Records if applicable
Individualized Education Program (IEP)/Evaluation Report (ER)/504 Plan

Pre-School Program | We currently have a waitlist for the 2024 - 2025 Preschool program. Register for the 2024-2025 UFRSD Preschool program waitlist or to be on our mailing list. |
K through 4th Grades | Kindergarten Registration for 2025-2026: Students may be enrolled into Kindergarten when they reach their 5th birthday on or before October 1st, 2025. 2025 Fall Kindergarten registration will begin February 24, 2025. Step 1: Parents are asked to complete the Online Pre-Registration Step 2: You will receive an email with links to upload the individual forms.
